You will work in a multidisciplinary team environment collaborating with the Diabetes Nurse Clinician, Health Professionals of the Red Lake Hospital, community agencies and the Family Health Team in the provision of a variety of health and prevention services. Diabetes Nurse Clinician
Dryden Native Friendship Centre Diabetes Program, DRYDEN
The Dryden Native Friendship Centre, a Northern Diabetes Health Network Member, is currently accepting applicants from interested, qualified individuals for the position of Diabetes Nurse Clinician who will provide combined diabetes education/management and foot care.The position is full time. This is a new program which will be part of a co-ordinated community based program designed to provide services to Aboriginal people in the Dryden area. The diabetes program involves ongoing education and clinical services to improve the quality of life of the population affected by diabetes. Position Responsibilities Provide diabetes education and care for adult clients with diabetes in an individual or group format. Ensure diabetes education and care is evidence-based and consistent with current Clinical Practice.Br>Guidelines as well as the philosophy, goals and objectives of the Diabetes Education Centre and the Dryden Native Friendship Centre. Responsible for maintaining knowledge base of current diabetes management strategies and clinical research development. In collaboration with the health care team, this position assumes responsibility for assessment, planning, implementation, care coordination and evaluation of clients referred to DNFC DEC. To plan, develop, implement and evaluate comprehensive, interdisciplinary teaching program and follow up for diabetes management. Collaborates with internal stakeholders and external community organizations/agencies to deliver effective diabetes care. Flexible work hours and some travel (valid Ontario Driver’s Licence required) will be essential in order to meet community and programming needs.
Qualifications: Must be a Registered Nurse holding current registration with the College of Nurses of Ontario. Confederation College Certificate in Diabetes Education or readiness to complete certification Certified Diabetes Educator, (CDE), or willingness to complete the requirements for certification. Demonstrated experience in diabetes care and education or relevant clinical experience. Demonstrate ability to set priorities and work independently and effectively in a fast paced environment. Self-directed, motivated team player, with a commitment to ongoing professional development. Current knowledge and sensitivity to patient multicultural needs. Experience working with Aboriginal people and communities and demonstrated understanding of Aboriginal culture values and perspectives an asset. Ability to communicate effectively via verbal, written and electronic means. Experience with Foot care &/or willing to attend specialized training to obtain certification.

Position Summary: Under the general supervision of the Clinic Coordinator and functional direction of the Executive Director, the Dietitian shall be responsible for the provision of clinical nutrition counselling and education services for the aboriginal clients of the Misiway Diabetes Program; provision of nutrition assessment of clients; provides recommendation to the interdisciplinary team for on-going treatment and evaluation; provision of diabetes education with the goal of maximizing self-management for the client. The Dietitian shall be responsible for the provision of general diabetes and nutrition information to the general aboriginal population in a consulting role.Roles and Responsibilities: Plans, develops, coordinates, implements and evaluates comprehensive, interdisciplinary teaching programs and follow-up for diabetes management. Participates in the efficient and effective operation of the Misiway Diabetes Program contributing to the planning, development, evaluation and revision of the purpose, philosophy, goals, objectives and operating strategies of the program and ensuring consistency with the corporate and management philosophy and strategy. Counsels clients in regards to nutritional needs on an individual basis. Assesses referrals to determine complexity of care needs and educational approach. Contributes to the development and delivery of workshops related to nutrition and diabetes education needs of the clients. Fosters and maintains effective working relationships with all segments of the internal and external community including but not limited to the Northern Diabetes Health Network, local and regional health care facilities, First Nation communities and relevant First Nation organizations, and, the Canadian Diabetes Association. Consults with organization’s physicians, nurse practitioners and traditional healers regarding the clients as necessary. Participates in the case management process of the organization. Be committed to participate and acquire a working knowledge and understanding of the aboriginal culture, healing approaches and ceremonial practices. Maintains accurate, comprehensive statistics. Maintains relevant records, files and charts with pertinent nutritional information. Acts as a nutritional resource person for clients, families, other health professionals and support agencies. Participates in continuous quality improvement activities.
Qualifications: Bachelor’s Degree in Applied Nutrition, Home Economics or Human Ecology Completion of Dietetic Internship Member of Dietitians of Canada Certified Diabetes Educator or College Diabetes Education Certificate desirable Must possess excellent communication, organizational and people skills Working knowledge of the aboriginal culture and healing approaches is desirable
